서론
류마티스 관절염은 완해와 악화를 반복하는 전형적인 만성 질환이기 때문에 대상자들이
자신의 상태를 어떻게 조절하는가가 질병 상태 뿐 아니라 적응 상태에도 많은 영향을 미친
다고 보고되어 왔다(Brown & Nicassio, 1987 ; Lazarus & Folkman, 1984 ; 김인자, 1994 ;
전정자와 문미숙, 1996). 류마티스 관절염 환자들이 사용하는 대처 방법을 면담을 통하여 조
사한 연구에서는(Abraido-Lanza, Guier, & Revennson, 1996) 활동 참가, 종교적 의존, 사회
적지지 추구, 약물과 물리 치료, 질병에 대하여 신경쓰지 않음, 긍정적인 재평가, 가족에 대
한생각, 휴식, 사회적 비교와 같은 방법들을 사용 한다고 보고하였다. 그러나 이 연구에서는
다양한 대처 방법들이 환자들의 적응 상태에 미치는 영향에 대하여는 조사하지 않았다.
한편 만성 질환자의 대처 유형과 적응과의 관계에 관심있는 여러 학자들은 다양한 방법으
로 대처 유형을 분류하여 대처 유형과 적응 상태와의 관계를 조사하였다. 대처 유형에 관계
없이 여러 대처 유형을 사용한 정도와 적응과의 관계를 본 연구들에서는 여러 대처 방법을
많이 사용할수록 적응 상태가 좋지 않았다고 보고하였다(Frank, 1987 ; Folkman 등, 1986 ;
김인자, 1995). 적극적 혹은 소극적 대처 방법으로 분류하여 조사한 연구들에서는(Brown &
Nicassio, 1987 ; Brwon, Nicassio, & Wallston, 1989) 일관되게 적극적 대처 방법이 적응
상태에 좋은 영향을 미친다고 하였다. 그러나 CSQ(Coping Strategies Questionnaire)와
WOCC(Ways of Coping Checklist)를 사용하여 조사한 연구자들은 요인 분석을 통하여 다
양한 대처 유형별 적응 상태를 조사하였는데 정보 추구는 대부분의 연구에서 긍정적인 적응
상태와 유의한 관계가 있었으나 인지적 조절은 음의 방향으로(Felton, Revenson, &
Hinrichsen, 1984 ; Vitalino et al, 1987, 1989) 혹은 양의 방향으로(Mann & Zautra, 1990 ;
Rosenstiel & Keefe, 1983) 적응 상태를 설명하여 서로 상반 된 결과가 제시되었다.
The effects of coping on physical and psychosocial adaptation in the 297 patients with
rheumatoid arthritis were Investigated. The coping methods were divided Into compliance
of medical regimens, self-control activities, and cognitive control. The effects of these
coping methods was analyzed with stepwise regression. The physical adaptation is found
to be significantly affected by cognitive control, self-control activities, and compliance of
medical regimens In order. Compliance of the medical regimens affects negatively the
physical adaptation. And psychosocial adaptation is significantly affected by the cognitive
control. The analysis of these results by illness duration showed the distinct differences.
That is in the patients who experienced the disease less than 48 months, the physical
and the psychosocial adaptation were significantly affected only by the cognitive control.
But in the patients who experienced the disease more than 96 months, the physical and
the psychosocial adaptation were significantly affected by self-control activities. Based
upon these results, it Is recommended that the nurses who care the newly diagnosed
patients emphasize on the positive side of the state. And the patients who are diagnosed
long ago need the nursing programs which teach and promote self-control activities.
Also it is suggested that the results of compliance must be considered rather than as
the result variable.
